Gesala Perez Junkera naiz eta gaur egun nire doktorego tesia burutzen ari naiz Gluten3S (Gluten: Osasuna, Segurtasuna, Gizartea) ikerketa taldean. Taldea, Farmazia Fakultateko Farmazia eta Elikagaien Zientzien departamentuan dago.

Institutuaeta gero, nire ibilbide akademikoa Euskal Herriko Unibertsitatean hasi nuen 2015ean eta geroztik, zentru bereko ikasle izaten jarraitzen dut. Lau urte eta gero, 2019an, Giza Nutrizio eta Dietetikako gradua lortu nuen. Lau urte horietan zehar izandako bi gertaera garrantzitsu aipatuko nituzke: Mexikoko Puebla de Zaragoza hirian urtebeteko egonaldia burutu izana eta graduko praktika tutelatuak eta praktika bolondresak, gaur egun kide naizen Gluten3S ikerketa taldean burutu izana.Elikadura eta Osasuna Masterra 2019ko urrian hasi eta 2020ko uztailean titulua eskuratu nuen. Master amaierako lana, gaur egun nire tesia denaren interbentzio pilotua izan zen. 2020ko udan, Eusko Jaurlaritzak eskaintzen duen Ikasiker beka-onuradun izan naiz ikerketan jarraitzeko aukera izanik eta urte bereko irailean tesia burutzen hasi nintzen. Pertsonalki, posible dudan bakoitzeannire ikasketekin edota nire ikerketa lerroarekin zerikusia duten formakuntza kurtsoetara noa parte hartzaile edo entzule gisa. Honetaz gain, herrialde desberdinetan egon niaz ingelesa ikasten.

Beti gustatu izan zait elikadurarekin zerikusia izan dezakeen edozertaz irakurtzea edo ikastea, eta gaur egun, nutrizionista-dietista izanik, badakit elikadura egokia jarraitzea ezinbestekoa dela gure osasun egoera egokia mantentzeko. Orain,  osasuntsu egoteari geroz eta garrantzia gehiago ematen diola sumatzen dut, baina nire ustetan oraindik badago zer ikertu elikadura-munduari dagokionez eta uste dut, landu beharra dagoela egindako aurkibide horiek nola helarazten zaizkion gizarteari ere; ikerketa mundutik gizarterako transferentzia egokia izan behar da. Hortaz, esan dezaket, biziki gustuko dudan arlo baten inguruan gizarteari lagundu ahal izateak motibatu nauela ikerketan jarraitzera eta gauzak berriak aurkitzeko gogoz egotera.

Aurretiaz aipatu bezala, Gluten3S taldeko kidea naiz. Taldearen helburua pertsona zeliakoen bizi kalitatea hobetzea da eta hurrengoak dira gaur egungo hiru ikerketa lerroak: 1. Elikagaien glutenaren eta intereseko beste molekula batzuen edukia aztertzea eta elikagaien garapenean osagai berrien erabilera ikertzea; 2. Glutenik gabeko dietarenoreka eta segurtasuna aztertzea eta sustatzea eta 3. Zeliakoen eta biztanleria orokorraren nutrizio-hezkuntza.

Ibilbidean zehar, Gluten3S ikerketa taldeak, zeliakoentzat esklusiboki diseinatuta dagoen software dietetiko bat sortu zuen. Bertan, 700 elikagai baino gehiagoren datuak biltzen dira. Graduko praktiketan, aipatutako softwarea osatu genuen aurretiaz neurtuta ez zeuden beste molekula batzuk laborategian teknika bereizgarri batekin neurtuz. Analisi horretan bildutako datuak softwarean gehitu ziren. Softwareari esker, Espainiako populazioren FODMAP kontsumoa determinatu ahal izan da, 2020ko abuztuan, lorturiko datuei esker, inpaktu altuko aldizkari baten artikulu bat argitaratuz.  Lorpen honetaz gain, nutrizio-hezkuntza arloko kongresuetan ere aurkeztu dira taldeak egindako beste interbentzio edota materiala ere.

Nire tesi proiektua “Glutenik Gabeko Dietaren abordatze integrala: oreka, segurtasuna eta inklusioa lantzen EAEko zeliako eta glutenarekiko populazio sentikorrean” da. Era laburrean azalduta, Gaixotasun Zeliakoaren (GZ) eta glutenarekiko sentikortasunaren (GS) tratamendu bakarra, bizitza osoan zehar glutenik gabeko dieta (GGD) zorrotz baten jarraipena egitean datza. GGDak karbohidratoak gutxiago kontsumitzea ekar dezake, eta horrek, dieta orekatua betetzeko zailtasun gehigarria dakar, eta, beraz, elikadura-desorekak izateko arriskua. Banakako  aholkularitza dietetikoa desoreka horiek saihesteko laguntza izan daitekeen arren, pertsona horiek ez dute zerbitzu indibidualizatu hori izaten, eta horrek tratamenduaren eraginkortasuna arriskuan jartzen du. GZaren forma klasikoan eta GSn, glutena hartzeak eragindako urdail-hesteko sintomatologia da nagusi. Pentsa liteke proteina hori dietatik kentzeak sintoma horiek erabat kanporatzea dakarrela; hala ere, egiaztatu da pazienteen %30etan ez duela horrelakorik egiten, eta, beraz, oso interesgarria da sintomak eragin ditzaketen beste konposatu batzuen inguruko ikerketa, hala nola FODMAPak. Aurrekoa kontuan hartuta, proiektu honen helburua, oro har, G-SZ pertsonen bizi-kalitatea hobetzea da. Horretarako, esku-hartze dietetiko pertsonalizatua diseinatu da, nutrizio-hezkuntzarekin koordinatuta, sintomatologiarekin erlaziona daitezkeen faktoreak identifikatzeko eta GGDren oreka hobetzeko, modu horretan, patologia horri lotutako arazo potentzialak saihesteko ere. Interbentzioan parte hartuko dutenak Euskal Autonomia Erkidegoko adin-pediatrikoko pertsonak izango dira. Adin txikiko pertsona-talde hau izango da gizartean oihartzuna zabalduko duena eta hauek, aldi berean, ezinbesteko dute osasun-egoera egokia mantentzea eta dietan nutrienteen hartzeegokia izatea hazkuntzan eta garapenean arazorik izan ez dezaten, eta epe luzera, honekin lotura izan dezaketen gaixoak eduki ez ditzaten.

Datozen urteetan, ikerketan segitzeko asmoa dut, betiere elikadurarekin eta osasunarekin zerikusia duten arloak etagauza berriak ikertzeko aukera baztertu gabe. Interbentzioak Euskal Autonomia Erkidegoko adin pediatrikoko pertsonak bilduko ditu, baina gustatuko litzaidake nire ikerketa ibilbidearen zatiren bat atzerrian egitea. Aberasgarria da beste lurralde batzuetan zer eta nola ikertzen den jakitea, eta aldi berean, guk, hemen egiten duguna ere munduan zehar zabaltzeko aukera izatea.

Soy Gesala Perez Junkeray actualmente realizo mi tesis doctoral en el grupo de investigación Gluten3S (Gluten: Salud, Seguridad, Sociedad). El grupo se encuentra dentro del departamento de Farmacia y Ciencia de los Alimentos de la Facultad de Farmacia.

Tras finalizar el instituto, inicié mi carrera académica en la Universidad del País Vasco en 2015 y desde entonces sigo siendo alumna del mismo centro. Cuatro años después, en 2019, obtuve el Grado en Nutrición Humana y Dietética. A lo largo de esos cuatro años me referiría a dos hechos importantes: haber realizado una estancia de un año en la ciudad mexicana de Puebla de Zaragoza y haber realizado las prácticas tuteladas y voluntarias de grado en el equipo de investigación Gluten3S al que pertenezco en la actualidad. Comencé a estudiar el Máster en Nutrición y Salud en octubre del 2019 y en julio de 2020, obtuve el título. El trabajo finde máster, fue la intervención piloto de lo que hoy día es mi tesis. En verano del 2020 fui beneficiaria de una beca Ikasiker concedida por el Gobierno Vasco. En septiembre del mismo año empecé con mi tesis. Personalmente, cada vez que puedo voy como participante o como oyente a cursos de formación relacionados con mis estudios o con mi línea de investigación.

Siempre me ha gustado estudiar a cerca de cualquier cosa que tuviera que ver con la alimentación, y hoy en día, siendo nutricionista-dietista, sé que seguir una alimentación adecuada es imprescindible para mantener un estado óptimo de salud. Además, ahora la gente cada vez da más importancia a la salud, pero creo que todavía hay que investigar en ello y creo que hay que trabajar en cómo se trasladan esos descubrimientos a la sociedad; la transferencia del mundo de la investigación a la sociedad debe ser adecuada. Por lo tanto, puedo decir que el poder ayudar a la sociedad en torno a un ámbito que me gusta mucho me ha motivado a seguir investigando y a tener ganas de estudiar cosas nuevas.

Como ya se ha mencionado anteriormente, formo parte del grupo Gluten3S. El objetivo del grupo es mejorar la calidad de vida de las personas celíacas y las tres líneas de investigación actuales son las siguientes: 1. Analizar el contenido de gluten alimentario y otras moléculas de interés e investigar el uso de nuevos componentes en el desarrollo de alimentos; 2. Analizar y promover el equilibrio y la seguridad de la dieta libre de gluten y 3. Educación nutricional celíaca y de la población general.

Durante el recorrido, el grupo de investigación Gluten3S creó un software dietético diseñado exclusivamente para celíacos. En ella se recogen los datos de más de 700 alimentos. En prácticas de grado, se completó el software mencionado midiendo con una técnica característica otras moléculas no medidas anteriormente en el laboratorio. Los datos recogidos en este análisis se incorporaron al software. El software ha permitido determinar el consumo de FODMAP de la población española y gracias a los datos obtenidos, se ha publicado un artículo en una revista de alto impacto en agosto de 2020. Además de este logro, también se han presentado en congresos de educación nutricional otras intervenciones y/o material realizado por el grupo.

Mi proyecto de tesis es “Abordaje integral de la Dieta Sin Gluten: trabajando el equilibrio, la seguridad y la inclusión en la población sensible a la celiaca y gluten de la CAPV”. Explicado brevemente, el tratamiento único de la Enfermedad Celíaca (EC) y de la sensibilidad al gluten (SG) consiste en el seguimiento estricto de una dieta sin gluten (DSG) a lo largo de toda la vida. La DSG puede suponer un menor consumo de hidratos de carbono, lo que supone una dificultad añadida para cumplir una dieta equilibrada y, por tanto, un riesgo de desequilibrios alimentarios. Aunque el asesoramiento dietético individual puede ser una ayuda para evitar estos desequilibrios, estas personas no cuentan con este servicio individualizado, lo que pone en peligro la eficacia del tratamiento. En la forma clásica de la EC y en la SG predomina la sintomatología gastrointestinal causada por la ingesta de gluten. Se podría pensar que la retirada de esta proteína de la dieta supone la eliminación total de estos síntomas, sin embargo, se ha comprobado que en el 30% de los pacientes no lo hace, por lo que es muy interesante la investigación sobre otros compuestos que pueden provocar síntomas, como los FODMAP. Teniendo en cuenta lo anterior, el objetivo de este proyecto es, en general, mejorar la calidad de vida de las personas EC-SG. Para ello, se ha diseñado una intervención dietética personalizada, en coordinación con la educación nutricional, para identificar los factores que pueden relacionarse con la sintomatología y mejorar el equilibrio de la DSG, evitando así también los problemas potenciales asociados a esta patología. Las personas participantes en la intervención serán personas de la edad pediátrica de la Comunidad Autónoma Vasca. Será este grupo de personas menores de edad el que se haga eco de la sociedad y éstas, a su vez, necesitan mantener un estado de salud adecuado y una ingesta adecuada de nutrientes en la dieta para que no tengan problemas en el crecimiento y desarrollo y a largo plazo para que no tengan enfermos que puedan estar relacionados con esto.

En los próximos años, pienso seguir investigando, sin descartar la posibilidad de investigar cosas nuevas que puedan tener que ver con la alimentación y la salud. La intervención reunirá a personas de la edad pediátrica del País Vasco, pero me gustaría mucho que alguna parte de mi trayectoria investigadora se realizara en el extranjero. Creo que es muy enriquecedor saber qué y cómo se investiga en otros territorios, y al mismo tiempo, yo, también tener la oportunidad de difundir por el mundo lo que hacemos aquí.

My name is GesalaPerez Junkeraand I am currently doing my PhD in the research group Gluten3S (Gluten: Health, Safety, Society)., which is located in the Department of Pharmacy and Food Sciences of the Faculty of Pharmacy.

After finishing High School, my academic career at the University of the Basque Country began in 2015 and since then I have been a student at the same center. Four years later, in 2019, I obtained a Degree in Human Nutrition and Dietetics. Throughout those four years, I would refer to two important events:completing a stay in the Mexican city of Puebla de Zaragoza by spending a year there and,completing the tutored and voluntary undergraduate internships in the Gluten3S research team to which I currently belong. Ibegan the Master’s in food and Health in October 2019 and in July 2020, I presented my final Master’s thesis and obtained the Master’s degree. The work that I just have mentioned was a pilot intervention of what is now my thesis. In the summer of 2020, I had the opportunity to continue researching for three months with the Ikasiker grant from the Basque Government and in September 2020 I began my PhD. Personally, whenever I can, I go as a participant or as a listener to training courses related to my studies or my line of research.

I am interested in reading or learning about anything related to nutrition, and today, as a nutritionist, I know that following a proper diet is essential to maintain our proper state of health and wellness. Besides, now I see that people are aware of the importance health has, but I think there is still research to be done in the world of nutrition and I think we have to work on how these indices are transferred to society; the transfer from the research word to society must be adequate. Therefore, I can say that being able to help society in an area that I like has motivated me to continue researching and wanting to find new developments.

As mentioned above, I am part of the Gluten3S group. The group aims to improve the quality of life of celiac people and the three current lines of research are the following: 1. To analyze the content of dietary gluten and other molecules of interest and investigate the use of new components in the development of foods; 2. To analyze and promote the balance and safety of the gluten-free diet and 3. Nutritional education for celiac disease and the general population.

During the process, Gluten3S research group created a dietary software designed exclusively for celiac people. In it, the data of more than 700 foods are collected. In my undergraduate practices, the mentioned software was completed by measuring other molecules not previously measured in the laboratory with a characteristic technique. The data collected in this analysis was incorporated into the software. Thanks to the data obtained the software has made it possible to determine the consumption of FODMAP in the Spanish population and published an article in a high-impact magazine in August 2020. In addition to this achievement, they have also been presented at educational conferences nutritional other interventions and/or material made by the group.

My PhD is titled “Comprehensive approach to the Gluten-Free Diet: working on balance, safety, and inclusion in the population sensitive to celiac disease and gluten in the Basque Country”. Briefly explained, the only treatment for Celiac Disease (CD) and gluten Sensitivity (GS) consists of strict adherence to a gluten-free diet (GFD) throughout life. GFD can lead to lower consumption of carbohydrates, which is an added difficulty in following a balanced diet and, therefore, a risk of dietary imbalances. Although individual dietary advice can help avoid these imbalances, these people do not have this individualized service, which jeopardizes the effectiveness of treatment. Gastrointestinal symptoms caused by gluten intake predominate in the classic form of CD and GS. It could be thought that the withdrawal of this protein from the diet supposes the total elimination of these symptoms, however,researchershave found out that in 30% of patients it does not, so the research on other compounds that can cause symptoms, such as FODMAPs is needed. Considering the above, the objective of this project is, in general, to improve the quality of life of G-SZ people. For this, a personalized dietary intervention has been designed, in coordination with nutritional education, to identify the factors that may be related to the symptoms and improve the balance of GFD, thus avoiding potential problems as well.

In the coming years, I plan to continue doing research, without ruling out the possibility of investigating new things that may have to do with nutrition and health. The intervention will bring together people of pediatric age from the Basque Country, but I would like some part of my research career to take place abroad. I think it is very enriching to know what and how is research in other countries of the words, and this way, I would have the opportunity to spread our knowledge in other countries.
